For Freshmen Students

• Php420 University Entrance Examination Fee

For Transferees

• Original Transfer Credentials/Honorable dismissal
• Original Certificate of Good Moral Character
• Original Certified True copy of Grades
• Letter of Application addressed to the Registrar Original
• Copy of NSO Birth Certificate
• Two (2) pieces 2x2 ID Picture
• UEE Result
• Php10,000 Downpayment
• Php420 University Entrance Examination Fee

For Senior High School Students

• Visit University Entrance Examination Online Application
• Php320 University Entrance Examination Fee

For International Students

Upon application for University Entrance Examination (UEE), student applicants shall be required to submit the following:

• Letter of lntent
• Three (3) copies of 1998 Revised Personal History Statement (P.H.S.)
• One (1) set of Official Transcript of Records/Scholastic Records duly authenticated by the Philippine Embassy or Consulate in the applicant's country of origin or legal residence
• One (1) set of Notarized Affidavit of Suppport and Proof of Adequate Financial Support duly authenticated by the Philippine Embassy or Consulate in the applicant's country of origin
• One (1) set of Certificate of Good Moral Character
• One (1) set of photocopy of Passport pages where full name, photo, and birth date appear, with last arrival stamp
• One (1) set of Police Clearance duly authenticated by the Philippine Embassy or Consulate in the applicant's country of origin or legal residence
• One (1) set of original of Quarantine Test (Medical Examination Result)
• Four (4) copies of 2 x 2 ID picture
• Php8,000 Acceptance Fee
• Php5,000 International Fee
• Php420 University Entrance Examination Fee
