Data privacy and consent form

You are invited to participate in the Adamson University Graduate Tracer Survey. Your participation is entirely voluntary. If you choose to participate, you will be asked to complete this survey, which will take approximately 5 to 10 minutes.

PURPOSE:

The Adamson University Graduate Tracer Survey is designed to collect information on the career and educational outcomes of our graduates. Specifically, we aim to assess how well our graduates are doing in their careers, including their employment status, relevance of their work to their field of study, starting salaries, and whether they are pursuing further education. Additionally, we seek to understand how our graduates are embodying the values of Character, Competence, and Charity in their professional lives.

The data collected will help Adamson University evaluate the effectiveness of our academic programs and identify areas for improvement. This information will also support decision-making processes for the University and provide prospective students with valuable insights.


DATA PRIVACY ACT:

In adherence to RA 10173 (Data Protection Act of 2012) and its Implementing Rules and Regulations, we are committed to safeguarding your personal information.


STORAGE, RETENTION, DISPOSAL:

Your personal data will be securely stored with both physical and technical security measures in place to protect paper and digital files. Retention: Personal data will be retained only for as long as necessary for the purpose of this survey. Disposal: Once the data is no longer required, we will take reasonable steps to securely dispose of it, ensuring it cannot be edited, processed, or disclosed further.


DATA PROTECTION:

Adamson University will implement reasonable and appropriate organizational, physical, and technical security measures to protect your personal data. Access to the collected data will be restricted to authorized personnel only.


Data Subject’s Rights.

  1. Right to be informed on the collection and processing of personal data through this consent form;
