Data privacy and consent form
Your participation in this graduate tracer survey is voluntary. You will be asked to sign a consent form should you decide to respond to this survey of Adamson University graduates. This survey will take you about 5 to 10 minutes to complete.
PURPOSE:
The Adamson University Graduate Tracer Survey is a research tool used to track the career and educational outcomes of its graduates. The purpose of the Adamson University Graduate Tracer Survey is to collect data on how well our graduates are doing in the workforce, whether they are employed in their field of study, what their starting salaries are, and whether they are pursuing further education. Moreover, this tracer survey seeks to gather information on how Adamson graduates are manifesting Character, Competence, and Charity, in their respective fields of specialization.
The data collected through this graduate tracer survey will be used by Adamson University to evaluate the effectiveness of programs and to identify areas where improvements could be made. Overall, the purpose of this graduate tracer survey is to provide valuable insights into the long-term impact of an educational program on its graduates and to help in decision-making both for Adamson University and prospective students.
DATA PRIVACY ACT:
In compliance with RA 10173 or the Data Protection Act of 2012 (DPA of 2012) and its Implementing Rules and Regulations, we are detailing the processing of the data you will provide.
STORAGE, RETENTION, DISPOSAL:
Personal data collected shall be securely stored, employing physical security to safeguard the paper files and technical security to protect the digital files. Adamson University will keep the paper and digital files only for as long as necessary. When the personal data is no longer needed, reasonable steps to securely dispose of such information that forbids further editing, processing, and unauthorized disclosure.
DATA PROTECTION:
The University shall implement reasonable and appropriate organizational, physical, and technical security measures to protect your personal data. Only authorized personnel shall have access to the data collected and processed.
Data Subject’s Rights.
Under RA 10173, the following are some of the rights the data subject may exercise, (for the complete list of rights see https://www.privacy.gov.ph/know-your-rights/):
- Right to be informed on the collection and processing of personal data through this consent form;
- Right to object on the processing of personal data or to restrict the processing of personal data upon request;
- Right to access the personal data collected and processed, upon request;
- Right to request for rectification of personal data;
- Right to withdraw his or her consent.
