University Entrance Examination Requirements
For Freshmen Students
Upon application for University Entrance Examination (UEE), student applicants shall be required to submit the following:
- 4 pieces of 2 x 2 ID picture
- Photocopy of Form 138
- Php 400 Entrance Examination Fee
For Transferees
Upon application for University Entrance Examination (UEE), student applicants shall be required to submit the following:
- 4 pieces of 2 x 2 ID picture
- Transfer Credentials or Honorable Dismissal
- Certificate of Good Moral Character
- Certified True Copy of Grades
- Letter of Application
- Php 400 Entrance Examination Fee
- Original Copy of NSO Birth Certificate
For Foreign Students
Upon application for University Entrance Examination (UEE), student applicants shall be required to submit the following:
- Letter of lntent
- 3 copies of 1998 Revised Personal History Statement (P.H.S.)
- 1 set of Official Transcript of Records/Scholastic Records duly authenticated by the Philippine Embassy or Consulate in the applicant's country of origin or legal residence
- 1 set of Notarized Affidavit of Suppport and Proof of Adequate Financial Support duly authenticated by the Philippine Embassy or Consulate in the applicant's country of origin
- 1 set of Certificate of Good Moral Character
- 1 set of photocopy of Passport pages where full name, photo, and birth date appear, with last arrival stamp
- 1 set of Police Clearance duly authenticated by the Philippine Embassy or Consulate in the applicant's country of origin or legal residence
- 1 set of original of Quarantine Test (Medical Examination Result)
- 4 copies of 2 x 2 ID picture
- Php 8,000 Acceptance Fee
- Php 5,000 Foreign Fee
- Php 400 University Entrance Examination Fee
